Leadership Academy Application

Applications are now being accepted for Leadership Springfield Academy Class XXI.  Deadline for submission is April 2, 2010.  To apply for the program, download the application below and email the completed application to Kim Everest, Administrative Coordinator at kim(at)springfieldchamber.com.  The "Parent/Guardian Release Form" must be completed by a parent/guardian as well as a school counselor/faculty member.  The release form may be mailed to Kim  c/o Leadership Springfield, PO Box 1687, Springfield MO, 65801 or faxed to 417-862-1611.  An application submitted without the release form will not be considered.
